Please use this as your reference point on obtaining medical treatment and the actual coverage benefits of your insurance plan. The policy certificate is the overriding document for all benefits and wording for your insurance plan.

Insurance ID Card
If for any reason you have lost or cannot locate your insurance ID card please contact your exchange provider who will be happy to provide you with a new ID card pre-filled with all your information. If you need your ID card urgently, please download a copy and fill in your information. It is vital that you carry your ID card with you at all times so please make sure you have a copy.
